What belongs in a risk register?
The risk, its likelihood, its impact, an owner, and the mitigation. A risk without a named owner is an observation, and observations don't get managed.
How does RAG status get decided?
From exposure — likelihood multiplied by impact. The template sorts by exposure so the register opens on what matters rather than on whatever was entered first.
How often should the register be reviewed?
Monthly for a business-as-usual register, fortnightly on an active project. The value is in the review cadence; a register written once and never revisited is documentation, not risk management.
